Scheduling your First Appointment
Thank you for considering our practice. We look forward to speaking to you. When calling to schedule your first appointment, it's important that we gather information to provide you the best visit possible. Please have the following information available when you call to make your first appointment:
Name, address, home, cell, and work phone numbers.
Insurance information (please have your card available)
Information about your involved foot including when symptoms began.
Most appointments can be scheduled within 24hrs of your phone call. Appointments may be delayed if the physician you are requesting is out of the office or there is a specific health condition to be considered.

What to Expect During Your First Appointment
Please arrive 15 minutes before your scheduled appointment and 20 minutes if you have not completed your new patient information online. Our office is located on the 1st floor in Suite 190 of the Glenbrook Hospital Medical Professional Building.
As you check in, you will be asked to provide and/or verify your information. Please make any corrections regarding your personal and insurance information at this time. If you have not pre-registered, you will be asked to fill out your registration forms. We recommend that you download and complete the registration forms prior to your appointment.
After registration, you will be taken to an examination room, where you will give your medical history and explain your foot condition. If needed, x-rays will be taken. You will be examined and consult with your physician to determine the best course of action for your foot/feet.
Your next appointment, if needed, will be scheduled as you check out.
If surgery is necessary, a surgery date will coordinated with a Medical Assistant and the hospital. Pre-operative, surgical, and post-operative procedures must be explained to you and your caregiver before proceeding surgery.
